This is a hardcover book, Three Wishes by Barbara Delinsky, and this copy is autographed! Nice. :)

Here's a synopsis of the story from the publisher:

"When waitress Bree Miller is hit by a car driven by novelist Tom Gates, she has a near-death experience during which she is promised three wishes. Once the wishes are granted, though, she'll die for good. When Bree wakes up, Tom is at her bedside to care for her, and they eventually fall in love. But the three wishes loom over the couple's happiness, and Bree will soon have tough decisions to make."
