This is the original, vintage 45 rpm release of David Seville and the Chipmunks', "Alvin for President". Flipside: "Sack Time". Record label Liberty F-55277. Comes with original pic sleeve.
Condition of record is VG (very good or better), having light surface scratches, one detectable to the touch. Pic sleeve grades VG+ or better havaing tear at the top as depicted in photo. Any question as to condition, write and I'll try to answer specifics.
