AutoPatch 4YDM.  This item powers up and appears to be in good physical and working condition.  I could not find a model number on the unit but it had a manual with it that said 4YDM.  I cannot verify this for sure so please take a close look at the pictures for verification of model number. 

I purchased this from a university.
