GE CDA15T Monitor Screen CDL1531A.  Please see this is some slight discoloration on the screen.  I didn't try to clean it off.  See photos.  This item has a damaged power port in the back.  Sometimes you have to wiggle and play with the power input to get it to turn on and stay on.  The correct way to repair it is to put a new power input socket in.  I am selling it for parts or repair.
