This is a Late 1990's New Era 59Fifty Royal Blue 3D White and Orange Emblem of the San Francisco Giants.  It is a (56 cm, size 7) Fitted Baseball Wool Hat.  It has the Gray Visor and still an intact plastic on the inside of the sweatband.
