This is a flat item with multiple sleeves that extend beyond the top when extended, but then you fold those extended parts down covering the sleeve. 

I don't know its original purpose, but it appears to be military.

The back has nothing on it, so it doesn't appear to be something that would attach to a belt, but there is a reinforced hole on the strap that could be used to hang it from something.
