Used by the Ohio Public Health Association. Issued by the National Tuberculosis Association. Includes Standard Weight Tables for Height and Age for both Boys and Girls. WIn this form, the child kept track of their weight during the crusade and over 12 weeks kept track of the chores performed , diet, health, bathing and more. Quite interesting.
