OEM Air Duct Heat Blend Actuator; this was unbolted off a a brand new heater box unit, not a mile on it.  It was of course installed in the box.  We wrote on the part to ID it. 

There are 5 actuators on the heater box - the part # on the part must match: HY32-19E616-DA
