This appears to be either a fleece baby blanket someone sewed trim and backing onto, or maybe it was a fleece fabric panel that someone finished with the trim and back.

It looks hand stitched. 

This does have some wash wear, including pilling to the front, and a few very small dots/spots on the trim and back, as shown.
