Either West Virginia Specialty Glass, Empoli or Fostoria, I have been unable to determine which one for sure

Stands 9" tall by 4 1/2" diameter . 

Stunning - photos do not show the beauty. I was unable to capture the rainbow effect and that dark area you see is from shadows.
